Introducción
En la prevención Primaria de la Fiebre
Reumática aguda es necesario una
correcta identificación y un adecuado
tratamiento antibiótico de la faringitis
por el estreptococo B – hemolítico del
grupo A (GAS).
El criterio estándar del dx de la
Faringitis por GAS es el cultivo de la

Introducción
La Penicilina es el tratamiento de
elección, debido a su costo-efectividad,
tiene un espectro de actividad estrecho,
y la resistencia del GAS a la Penicilina
no ha sido documentada

Introducción
Los individuos quienes tienen un ataque
de Fiebre Reumática, tiene un muy alto
riesgo de desarrollar recurrencia
después de una Faringitis GAS

Diagnóstico de Laboratorio de la Faringitis por GAS
La correcta diferenciación de la faringitis por
GAS de la faringitis causada por otros
patógenos basada en la historia y los
hallazgos clínicos es frecuente la dificultad
aún en los médicos con experiencia
Por lo tanto, algunas formas de confirmación
microbiológica, como el cultivo de garganta o
el test de detección rápida de antígenos
(RADT), es requerida para el dx de Faringitis

Cultivo de Garganta
El cultivo de garganta es el método
convencional para establecer el dx de
Faringitis por GAS y es el criterio
standard
Un cultivo de la garganta negativo
permite a los médicos negar la terapia

Test de detección rápida de antígeno
Muchos de estos test tienen un alto
grado de especificidad, pero su
sensibilidad en la práctica clínica puede
ser inaceptablemente baja. Por lo tanto

Test de anticuerpos estreptocócicos
Los títulos de anticuerpos
antiestreptocócicos reflejan eventos
inmunológicos pasados y no presentes
y por lo tanto no pueden ser usados
para determinar si un individuo con
faringitis y GAS en la faringe es una

un portador de estreptococos

Recomendaciones del tratamiento
La Prevención de la FR requiere de una
terapia adecuada para la Faringitis por GAS.
En la selección del régimen para el tto de
dicha faringitis, los médicos deben considerar
varios factores: incluyendo eficacia
bacteriológica y clínica, fácil adherencia al
régimen recomendado, costo, espectro de
actividad de la selección del agente y
potencial efectos adversos

Las fluoroquinolonas tradicionales (por
ejemplo: Ciprofloxacino, tiene limitada
actividad contra GAS y no debe ser
usado para tratar faringitis por GAS

Seguimiento del cultivo de garganta
Post-tratamiento el cultivo de garganta
de 2 o 7 días después de completar la

Falla al tratamiento
Los individuos sintomáticos quienes
continúan con GAS en sus faringes
después de un curso completo de
terapia puede ser retratado con el
mismo agente antimicrobiano, dar un
agente oral, o una dosis IM de PNC G
benzatínica, especialmente si hay pobre
adherencia a la terapia oral

Los agentes con cefalosporinas de
espectro estrecho, Clindamicina,
Amoxicilina – ácido clavulánico o la
combinación de PNC con Rifampicina,
son razonables en el tto de pctes con
faringitis GAS en quienes el tratamiento
inicial de PNC han fallado (Clase IIa,

Otros abordajes de Prevención Primaria
Aunque la vacuna para el GAS es costo
– efectivo debería ser la solución ideal,
problemas científicos han impedido el
desarrollo de tales vacunas

Hay estudios no controlados que
demuestran que la tonsilectomía es

La Prevención Secundaria de la FR, es
definida como la continua
administración de antibióticos
específicos para pacientes con previo
ataque de FR o una enfermedad de
corazón reumática (RHD) bien
documentada

Definición
El propósito es evitar la colonización o
la infección del URT con GAS, y el
desarrollo de ataque recurrente de FR
La profilaxis secundaria es mandatoria
para todo pcte quien ha tenido un
ataque de FR, con o sin enfermedad
cardiaca valvular reumática residual

Factores de Riesgo de la recurrencia de la FR
Diversos factores pueden influir en el riesgo de recurrenca de la FR e incluyen:
– La edad del paciente
– La presencia de RHD
– El tiempo transcurrido desde el último ataque
– El número de ataques previos
– Una historia familiar de FR / RDH
– El estatus socioeconómico y educacional del individuo
– El riesgo de infección estreptocócica en el área
– Si el pcte esta dispuesto a recibir inyecciones
– La ocupación del pcte (escolar, profesor, médico, militar entre otros)

Situaciones especiales
La profilaxis con PNC para la
recurrencia de ataques de la FR debe
ser continuada durante el embarazo. No
hay evidencias de teratogenicidad
asociada con bencilpenicilina
benzatínica. Las drogas sulfas no están
recomendadas debido al potencial
riesgo sobre el feto

Situaciones especiales
Los pctes que tienen una válvula
proteica cardiaca permanecen
susceptibles de recurrencia de la FR,
pero debe haber precaución en
recomendar PNC G benzatínica IM para
pacientes con una válvula protésica que
recibe Warfarina u otro anticoagulante

Alergia a la PNC
La incidencia de Reacciones de
hipersensibilidad a la PNC ha sido bien
estimada en 2 – 5 %
La más común reacción alérgica es el
rash en la piel
La Anafilaxia es rara y ocurre en cerca
del 0.2 % de los casos

Alergia a la PNC
Debido a su pobre función cardiaca
estos pacientes son más susceptibles
de Reacción vaso-vagal y tienen un alto
riesgo de arritmias que comprometen la
vida . La Resucitación puede ser difícil

Test cutáneo de PNC
El test cutáneo de PNC es un método
aceptable y usualmente correcto para
determinar si una persona tiene riesgo
de tener una reacción inmediata a la
PNC

Test cutáneo de la PNC
Si un pcte tiene una convincente
historia de una severa reacción alérgica
inmediata a la PNC (oral o IM), el test
cutáneo no está recomendado y un
antibiótico no betalactámico debe ser
usado (por ejemplo eritromicina, sulfa)
