• La nefritis lúpica es la afectación visceral más frecuente del Lupus Eritematoso Sistémico
• Es el elemento individual más importante para establecer un pronóstico global de la enfermedad

EPIDEMIOLOGIA
• Mas frecuente en latinos, afroamericanos que anglosajones por aparentes factores socioeconómicos y ambientales.
• Afecta 10 mujeres : 1 hombre.• Prevalencia entre 4 y 250/100000 habitantes.• Puede presentarse a cualquier edad, mas
afectados entre los 16 y los 55 años.

• La NFL Involucra entre el 35 y el 90% de los pacientes con LES.
• La NFL es la forma de presentación inicial del LES es en 3 al 6 % de los casos.
• El diagnóstico es usualmente en los primeros 5 años del LES.

ETIOPATOGENIA
• Es el prototipo de la enfermedad de complejos inmunes.
• Complejos inmunes circulantes o los que se forman en el glomérulos
• Mecanismos propuestos: - Auto anticuerpos - Alteración apoptosis - Papel de los nucleosomas

In Panel A, circulating complexes of DNA and anti-DNA in blood are trapped in the basement membrane of glomerular capillaries, where they fix complement (not shown) and damage tissue. In Panel B, pathogenic subgroups of antibodies to DNA (or those antibodies bound first to DNA and then to nucleosomes) bind to components of glomerular-capillary basement membrane or to antigens trapped in the membrane. These antigens may include DNA, nucleosomes, heparan sulfate, and laminin. Complement is fixed and activated, causing tissue damage.

• La NFL tiene una presentación clínica muy variada.
• No existe una correlación lineal entre presentación clínica y tipo histológico.
• En un 20% de los pacientes puede presentarse en forma silente.

• Como no existe una correlación clínica lineal es necesario confirmar el diagnostico con Bx renal.
• La Bx renal nos da además información sobre el pronostico de la NFL

TRATAMIENTO
• No existe un tratamiento completamente satisfactorio.
• El tratamiento depende del tipo histológico y/o de las manifestaciones extra renales.
• Se debe tener en cuenta los efectos secundarios del tratamiento

OBJETIVOS TERAPEUTICOS
• Lograr una pronta remisión (remisión renal con primera línea de terapia en 81%).
• Evitar recaídas renales (30 – 50%).• Evitar ERC (10 a 20% en los siguientes 5 a 10
años)• Mínima toxicidad (altos a nivel metabólico
óseo con glucocorticoides).

• El tratamiento de los casos severos tiene 2 partes:
1) Fase de Inducción: Objetivo: Rescate de la masa renal
2) Fase de Mantenimiento: Objetivo: Minimizar recaídas

NEFRITIS LUPICA III/IV (KDIGO)
• Fase de Inducción ( 6 meses) - 3 Pulsos i.v. de 250 a 1000 mg 6-metil prednisolona - Prednisone oral (1 mg/Kg). - Micofenolato Mofetil ò - Bolos mensuales Ciclofosfamida (0.75 g/m2). - 6 Bolos quincenales de 500 mg de Ciclofosfamida
• Fase de Mantenimiento.( mínimo 1 año después de remisión)
- Prednisona oral (dosis minima). - Azatioprina ó MMF (ICN en intolerantes a lo previo) -Bolos Ciclofosfamida trimestral

• Si 3 meses después de FI hay deterioro de función renal: Cambio terapia o re biopsia
• Si después de 12 m no hay remisión completa: re biopsia antes de cambio de terapia.
• Si se empieza a retirar la terapia de mantenimiento y se deteriora la función renal: reiniciar la que funcionó
