Valoración del riesgo cardiovascular en el
peritrasplante
70% de los eventos clínicos adversos mayores en el primer año postrasplante son de origen cardiovascular.
La obesidad tanto del donante como del receptor supone un riesgo añadido de complicaciones postquirúrgicas.

– 5 años: 9% – 10 años: 25%
• Factores de riesgo cardiovascular en los que sobrevivieron 10 años postTOH: – Hipertensión: 47.6% – Diabetes mellitus: 13.6% – IMC>28: 13% – Dislipemia(CT>220mg/dL): 35%
• Riesgo cardiovascular: aumento multifactorial
– alteraciones metabólicas: • Hipertensión: 65-70% (ciclosporina > tacrolimus) • Diabetes Mellitus: de novo 5-35%(tacrolimus > ciclosporina) • Obesidad (ciclosporina) • Dislipemia : CT 16-43% // TG 40-47%. (ciclosporina > tacrolimus)
– factores genéticos (TM6SF2) – medicación inmunosupresora – Indicación del TOH: NAFLD.

• Síndrome metabólico postTOH:

• Factores de riesgo para recurrencia:
– Obesidad: 24% 4 meses postTOH 41% a los 3 años.
– Glucocorticoides: Obesidad, HTA.
– Ciclosporina: lipoproteínas aterogénicas
– Tacrolimus: Resistencia insulínica
– NAFLD aumenta metS x 3.4
