• Health-care costs are growing will, if unchecked, possibly

bankrupt us.

• Cancer drugs, currently comprise eight out of the top ten most

expensive drugs covered by Medicare.

• Cancer is in total responsible for only five percent of current

health-care expenditures, cancer care costs are expected to

grow from $125 billion in 2010 to a projected $175 billion

(using 2010 dollars) in 2020—a 40 percent increase

• Projections from the federal government indicate that health-

care spending will account for nearly one-fifth of the

economy in 2021.

• Some of the most expensive and commonly prescribed

medical tests and treatments are not evidence-based and

offer very little proven value to the patient.

• Only is that high-quality care—regardless of whether it is

expensive or not—often turns out to be less costly in the long-

term, in part because it is the most effective treatment, given

at the right time, for the appropriate patient.

• We need to find ways to assure the best use of limited

resources,

every patient.

the hallmarks of high-quality oncology care

collectively signal high-quality care in common cancers.

• ASCO’s Quality Oncology Practice Initiative (QOPI), launched

in 2006,

• Taking a cue from industry, where dramatically reduced error rates in

flying airplanes and building cars have been accomplished by

standardizing processes and strict use of checklists, incorporating

more standards in medical care will reduce error and save lives.

• All oncologists should be aware of these standards and should push

our offices and institutions to implement them

• Cancer science and information technology are advancing rapidly,

but the way we care for patients today cannot fully capitalize on

those advances.

• The proliferation of scientific results and novel treatments is a

growing challenge as we enter the era of highly personalized cancer

care.

• Patients are increasingly presenting with “rare cancers,” more

narrowly defined by their molecular characteristics, sometimes

making the best course of treatment unclear.

• Oncologists need real-time decision support to help them provide

the most effective treatments tailored to their patients’ unique

biology and tumors.

• Today, we know very little about most patients with cancer – from

the molecular characteristics of their tumors to the outcomes of their

treatments – because these details are locked away in unconnected

electronic and paper records.

• Cutting edge health IT platform, will use “big data” to revolutionize

how we, as oncologists, care for people with cancer.

• Designed to aggregate and analyze an almost infinite web of real-

world cancer care data.

• To assemble all of that information in a learning computer network

assessment and reporting based

on established guidelines

(e.g., any EMR, pharmacy,

• The soaring cost of cancer drugs is under scrutiny from institutional

decisions to not offer certain expensive new therapies to

commentaries from leading oncologists

• Drug costs are only one of a multiplicity of causes (technological

innovations in surgery and radiation therapy, hospitalizations, futile

cancer care provided at the end of life) for rapid rises in the costs of

cancer care and the financial burdens they collectively represent.

• Oncology providers will need to adapt to growing demands to

demonstrate quality, efficiency and transparency

• Oncology community will need to address the spiraling costs of new

cancer therapies and tests.

• Oncologists have not only a role, but indeed a responsibility to help

address and manage the issue of high drug costs.

• El MSIII ha iniciado el proyecto Compromiso por la Calidad

de las Sociedades Médicas en España.

• Enmarcado en las actividades de la Red Española de

Agencias de Evaluación de Tecnologías y Prestaciones del

Sistema Nacional de Salud y coordinado de forma conjunta

por la Subdirección General de Calidad y Cohesión, por el

Instituto Aragonés de Ciencias de la Salud, del que forma

parte GuíaSalud y por la SEMI.

• El objetivo principal del mismo es disminuir el uso de

intervenciones médicas innecesarias, entendiendo por

tales aquellas que no han demostrado eficacia, tienen

efectividad escasa o dudosa, no son coste-efectivas o no son

prioritarias.

• Los objetivos secundarios son la reducción de la variabilidad

en la práctica clínica, contribuir a difundir entre los médicos y

los pacientes la necesidad y el compromiso con un uso

adecuado de recursos sanitarios y, finalmente, promover la

seguridad clínica evitando la iatrogenia secundaria.

• Requiere

– la revisión de guías clínicas basadas en la evidencia,

con un soporte metodológico adecuado (GuíaSalud),

– la revisión por parte de un grupo de expertos designados

por cada una de las sociedades de estas evidencias

preseleccionadas

– la inclusión de sugerencias de otras fuentes,

– la realización de una técnica Delphi con una escala de

puntuación y jerarquización.

• Al final cada sociedad propondrá 5 recomendaciones de no

hacer, seleccionadas según un criterio de priorización basado

en su impacto clínico y económico y contarán con el aval de

cada sociedad.

• Las conclusiones serán seleccionadas y avaladas por

las propias sociedades médicas, pues estas encarnan

el rigor y la actualización del conocimiento científico.

• Están dirigidas a los profesionales, a los pacientes

y a las autoridades y directivos sanitarios.

• Tienen un gran valor de orientación, de modificación

de la praxis, al ser prácticas desaconsejadas, con

impacto en la reducción de costes o en un uso

alternativo de los recursos sanitarios

• GuíaSalud es un organismo del Sistema Nacional de

Salud (SNS) para promover la elaboración y uso de

Guías de Práctica Clínica (GPC) y otras herramientas y

productos basados en la evidencia científica.

• El Instituto Aragonés de Ciencias de la Salud (IACS) es

su Secretaría Técnica.

• Promover la elaboración, adaptación y actualización de Guías de

Práctica Clínica (GPC) y otros instrumentos basados en la evidencia

científica, acordes a las necesidades del Sistema Nacional de Salud

(SNS) y relevantes para el trabajo de los profesionales sanitarios.

• Facilitar el acceso y la utilización de un número significativo de

guías y otros instrumentos y recursos basados en la evidencia para

el apoyo a la toma de decisiones de los profesionales.

• Favorecer la implementación y utilización de las GPC.

• Evaluar el impacto en salud y la mejora continua de la calidad

en relación a la aplicación de las GPC en el SNS .

• Promover la formación para la elaboración, implementación y

evaluación del impacto de las GPC y desarrollar herramientas de

ayuda a los profesionales sanitarios.

• Es necesario conocer que resultados en salud tenemos y para ello

hemos de medir la calidad.

• La información sobre el nivel de calidad permite aplicar mejoras en los

puntos débiles y seleccionar las acciones más efectivas.

• Los clínicos y las SSCC debemos definir cuales son las estrategias que

aportan valor para el paciente y definir herramientas que nos permitan

medir la calidad asistencial.

• Los indicadores de calidad son herramientas para mejora de la calidad

pero nunca deben ser considerados una finalidad en sí mismos.

• Disponer de información válida y fiable es imprescindible pero también

lo es actuar en función de resultados
