OBJETIVOS
•Discutir la anatomía quirúrgica, biomecánica ysemiología del hombro en función de lasfracturas de húmero proximal.•Analizar la vascularización de la cabeza humeral.• Determinar los factores pronósticos en funcióna la necrosis avascular de la cabeza humeral•Opciones de tratamiento.
INTRODUCCIÓN
HOMBRO
EPIDEMIOLOGÍA• Representa del 4% al 5% de todas las fracturas. Segunda
más frecuente en miembro superior• Tercera fractura más frecuente en mayores de 65 años• Mayoría de las fracturas no desplazadas se pueden
tratar ortopédicamente• Relación 3:1 :: Mujeres:Hombres.• En ancianos debido a la osteoporosis.• Pacientes jóvenes pora accidentes de alta energía.• El 20% de las fracturas desplazadas son quirùrgicas.

•Los patrones básicos de fractura: 2, 9, 10, 11 y 12. ( 79%).
fractura de cuello anatómico con calcar corto la necrosis asciende al 97% !!! sin fulcro interno

•Angulación en el Desplazamiento De La Cabeza (62%).

Indicaciones:1. Fractura y fractura/luxación de cuatro fragmentos con tuberosidades
osteoporóticas y conminutas
2. Fracturas conminutas con destrucción de la superficie articular de más del 40%.
3. Manguito de los rotadores atrófico con degeneración grasa
4. Fracaso de osteosíntesis en fracturas o fracturas-luxaciones irreducibles y en casos de consolidación viciosa de las tuberosidades.

Contraindicaciones:1. Infección activa.
2. Disfunción del deltoides (neurológica o estructural) ya que es el único motor funcionante de la artroplastia.
3. Déficit de capital óseo glenoideo que impida la colocación del implante.
4. Pacientes jóvenes (exceptuando procedimientos de rescate).

Conclusiones.
Edad, calidad ósea y patrón de la fracturason las consideraciones básicas para indicar eltratamiento
La estabilidad de la reducción y laviabilidad de la cabeza humeral son losfactores principales a considerar
Si la estabilidad es precaria o el hueso demenor calidad las placas bloqueadas suponenun gran avance
En situaciones de inestabilidad y cabezainviable la prótesis con reposición anatómicade las tuberosidades ofrece resultadosfuncionales consistentes
