Ticagrelor inhibits adenosine uptake via
the nucleoside transporter ENT-1
• In addition to its antiplatelet
effects via the P2Y12 receptor,
ticagrelor also enhances local
adenosine response by inhibition
of the ENT-1 transporter1–5
• Enhanced local adenosine
response may result in:
– Additional inhibition of platelet
aggregation/activation*3
– Cardioprotection6
– Vasodilation5,7,8
– Modulation of inflammation
– Dyspnoea7

Estudio PLATO
Resumen resultados
• En el Estudio PLATO, ticagrelor redujo significativamente el criterio compuesto de
muerte CV, IM o ictus frente a clopidogrel a 1 año (1,9% RRA, 16% RRR, P<0,001,
NNT=54).
• Ticagrelor redujo significativamente el IM frente a clopidogrel (16%).
• Ticagrelor redujo significativamente la mortalidad CV frente a clopidogrel (1,1% RRA,
21% RRR, p=0,001).
CABG)

in patients with recent MI, recent stroke or symptomatic PAD compared with ASA
after a mean follow-up of 1.9 years
• CHARISMA: Post-hoc subgroup analysis[Bhatt 2007]
– Clopidogrel + ASA continued to reduce the risk of CV events beyond 1 year of
treatment in patients with symptomatic CV disease
• TRA2oP-TIMI 50: Pre-specified subgroup analysis[Scirica 2012]
– Vorapaxar added to ASA a thienopyridine reduced the risk of CV events after
3 years of treatment in patients with a prior MI
– Bleeding was increased but a significant overall net clinical benefit was observed
ASA, acetylsalicylic acid; CV, cardiovascular; DAPT, dual antiplatelet therapy; MI, myocardial infarction; PAD, peripheral artery disease.

PEGASUS-TIMI 54: Rationale and
study aim
• The use of DAPT has been shown to further reduce the risk of
ischaemic events versus ASA alone in patients with ACS[Yusuf 2001;
Sabatine 2005]
• However, the optimal duration of DAPT remains to be determined
• Although landmark trials showed a continued accrual of benefit
beyond the immediate period following the ACS, the approximate
duration of trials was only 1 year[Antman 2008; Yusuf 2001; Wallentin 2009]
• The PEGASUS-TIMI 54 study was designed to:[Bonaca In press]
– Investigate the efficacy and safety of ticagrelor in patients with a history of MI
(within 1–3 years) and at least one additional thrombotic risk factor
ACS, acute coronary syndromes; ASA, acetylsalicylic acid; DAPT, dual antiplatelet therapy; MI, myocardial infarction.
