• Incidencia de 0,2 a 0,3% de la población total por año

del tercer ventrículo, compresión u obliteración de las cisternas de la base

contralateral), y/o que presenten además HIC refractaria o deterioro del nivel de conciencia.

Higromas o Hidromas subdurales
• Colecciones de líquido subdurales.• Hipodensos en TC.• Pueden aparecer después de traumatismo
cefálico sin hematoma previo. • Puede ser agudo (poco frec.),se produce por
pequeños desgarros aracnoideos, esto permiten que el LCR se acumule en el espacio subdural.
• Crónicos (mas frec.) se producen por la reabsorción de un hematoma preexistente.
• Es más frecuente que los higromas subdurales se desarrollen entre el segundo día y segunda semana.

HEMORRAGIA SUBARACNOIDEA
• El 50-70% se producen por ruptura de aneurismas intracraneales.
• Menos frec. traumatismos, hemorragias intraparenquimetosas primarias, MAV, tumores hemorrágicos, discrasias sanguíneas, angiopatía amiloide, complicaciones del embarazo.

• En las primeras 24hs. La TC muestra en el 95% e los casos sangre en cisternas y espacios suaracnoideos.
• La TC disminuye su sensibilidad con el tiempo 80% al día 3, 70% día 5, 50% día 7 y 30% dia 14.
• En agudo la RM no es de gran utilidad ya que la unica secuencia que nos muestra una imagen compatible con h. subaracnoidea es el FLAIR (HIPER en espacios correspondientes).
• El índice de resangrado es de 4-20% en las primeras 24hs. Y del 1-2% a las 2 semanas.

Clasificación • Lineales: poca relevancia clínica, no así
las lesiones asociadas (contusiones, desgarros).
• Diastásicas: separación de suturas.
• Conminutas: múltiples fragmentos.
• Deprimidas: por lo gral. son conminutas.
Hay que prestar especial atención a las fracturas abiertas y a las que atraviesan los senos ya que conllevan un mayor riesgo de infecciones intracraneal.
