CASO CLINICOPACIENTE MASCULINO 5 SEMANAS DE
EDAD, PESO 5 KG , SE ALIMENTA CON LECHE MATERNA CADA 30-60 MINUTOS, O CUANDO EL LACTANTE LO PIDE. VOMITA LUEGO DE CADA TOMA, A VECES EN CANTIDADES IMPORTANTES. MADRE SE LOGRA EXTRAER HASTA 4 ONZAS EN 5/10 MINUTOS CON LA MAQUINA EXTRACTORA DE LECHE. LA MADRE PONE AL NINO AL PECHO 15-20 MINUTOS. LOS VOMITOS NO SON BILIOSOS

CASO CLINICOPACIENTE FEMENINA 4 MESES DE EDAD,
CON HISTORIA DE VOMITOS POSTPRANDIALES, NO BILIOSOS, NIEGA HEMETEMESIS. DEPOSICIONES NORMALES. PESO: 6.5 KG, TALLA: 64 CM. LA MADRE LE DA LACTANCIA MATERNA 2-3 VECES POR DIA, Y FORMULA A BASE DE PROTEINA DE VACA 4 ONZAS CADA 3 HORAS (TOTAL 6 CHUPONES POR DIA).

50-60% de todos los lactantes de 0-3 meses regurgitan al menos 1 vez al dia.

Trials were identified by searching Cochrane Central Register of Controlled Trials (The Cochrane Library 2003), MEDLINE (January 1966 to 23 January 2003), EMBASE (January 1985 to 27 January 2003), and reference lists of articles. Searches in all databases were updated in April 2009.
Randomised studies (parallel or cross over) which evaluated thickened feeds, positional alternations or metoclopramide for the treatment of GER in children between the age of one month and two years who were developmentally normal

20 estudios (771 ninos tuvieron los criterios clinicos. 8 espesamiento de la formula, 5 con posicion anti-reflujo y 7 con metoclopramida.
Espesamiento de la formula reduce los sintomas de RGE
Elevacion de la cabecera de la cama no tuvo efecto en la reduccion de RGE
Metoclopramida reporto algunos beneficios en comparacion con el placebo en ninos con RGE sintomaticos, per debe sopesarse riesgo-beneficio .

PPIs do not always result in adequate acid suppression Adequate salivary secretions are essential for the
protection of the teeth and the oropharyngeal and esophageal mucosa.
