El vástago primario KAREY HA sincemento está fabricado en aleaciónde titanio forjado (TiAl6V4).
Vástago rectilíneo de sección cua-drangular dividido en dos porciones:
a) Proximal o metafisaria, con formatrapezoidal en los planos sagitaly frontal, para conseguir una estabi-lidad primaria tridimensional y evi-tar el hundimiento protésico.
b) Distal o diafisaria, subdimensio-nada progresivamente para dismi-nuir la rigidez del implante, mejorarel contacto con el hueso esponjoso,favoreciendo el crecimiento óseo yla estabilidad secundaria delimplante.
El subdimensionado progresivo dela porción distal evita el relleno dia-fisario disminuyendo el stressschielding.
El recubrimiento de HA se realizamediante la moderna técnica decañón de plasma (Air PlasmaSpraying), tiene un grosor de 130micras y se aplica en la totalidaddel implante.
Tanto el vástago recubierto con HApara su uso sin cemento, como elvástago cementado KAREY-C seimplanta con el mismo instrumental.
The cementless KAREY HA primarystem is manufactured in forgedtitanium alloy (TiAl6V4).
Straight stem of quandrangularsection divided into two portions:
a) Proximal or metaphyseal, trape-zoidal in shape on sagital and fron-tal planes, obtaining three-dimen-sional primary stability and avoi-ding component subsidence.
b) Distal or diaphyseal, progressi-vely subsized to reduce implant rigi-dity, improving contact with thecancellous bone, enhancing bonegrowth and secondary stability ofthe implant.
The progressive subsizing of thedistal portion to avoid diaphysealfilling, reducing the schieldingstress.
The HA coating is applied using thelatest technique of Air PlasmaSpraying, with a thickness of 130micron over the whole implant.
Both the HA coated stem for usewithout cement, and the KAREY-Ccemented stem are implanted usingthe same instrumentation.

Vía de abordaje · El objetivo de cualquier vía deabordaje es la visualización correcta del acetábulo ydel fémur proximal. La elección de la vía queda a cri-terio de cada cirujano según sus preferencias.
Surgical approach · The objective of any surgicalhip approach is correct visualization of the acetabu-lum and proximal femur. The choice of surgical hipapproach will be based on the surgeon's preferences
1 · Osteotomía del cuello femoralCon la plantilla femoral sobre la radiografía preopera-toria, determinaremos el nivel de osteotomía delcuello femoral. La osteotomía se realizará en ángulodiedro a 45º (Fig. 1).
1 · Femoral neck osteotomyWith the femoral template on the preoperative X-ray,we will determine the proper femoral-neck level ofresection. The osteotomy will be performed at a 45ºdihedral angle (Fig. 1).
2 · Preparación femoralSe inicia la apertura del canal con el cincel hueco(Fig. 2), extirpando hueso suficiente de la paredmedial del trocánter mayor en dirección paralela al ejefemoral. Se introduce la fresa cónica manual (Fig. 3), lo máslateral posible y paralelo al eje del fémur, para encon-trar el canal femoral.Con la raspa de menor tamaño montada en el mangoportarraspas (Fig.4), se inicia el raspado introducién-dola hasta la ultima línea de dientes. Se repite estaoperación aumentando sucesivamente el tamaño delas raspas de forma progresiva hasta contactar con lacortical endomedular. La anteversión se establece conestas maniobras de raspado.
2 · Preparation of the femurStart opening the femoral canal using a hollow boxchisel (Fig. 2), removing enough bone from the medialwall of the greater trochanter parallel to the femoralaxis.Insert the manual conical reamer (Fig. 3), as laterallyas possible and parallel to the femoral axis to find thefemoral canal.With the smallest rasp mounted on the rasp-handle(Fig.4), start the reaming, inserting it as far as thelast line of teeth. This operation must be repeatedincreasing rasp size sequentially until the endomedu-llar cortical chatter is found. The anteversion is esta-blished with these rasping operations.

3 · Reducción de pruebaCon la raspa introducida en el canal femoral se pro-cede a la reducción con las cabezas de prueba pararaspas, con el fin de seleccionar el largo de cuellonecesario (Fig. 5). Finalmente con la medida de cuelloelegida, comprobamos la estabilidad, la tensión y lalongitud de la extremidad.
3 · Trial reductionWith the rasp inserted in the femoral canal, carry outthe reduction with the trial heads for rasps, in order toselect the proper neck length (Fig. 5). Finally, once theneck length has been chosen, stability, tightness andleg length are tested.
4 · Inserción del componente femoralEl vástago definitivo corresponderá a la misma medi-da de la última raspa utilizada. Acoplamos el vástagoal mango introductor específico (Fig. 6), procediendoa la impactación hasta introducirlo a la profundidaddeseada, en este punto se retira el mango introductor.Con el impactor final se puede impactar el vástagodefinitivo de forma complementaria, si se considera-se necesario (Fig. 7). Finalmente colocamos la cabeza definitiva escogidaen el vástago y se reduce la articulación con la ayudadel impactor-reductor (Fig. 8).
4 · Femoral component insertionThe definitive stem will correspond to the same size asthe last rasp used. Mount the stem on the specificinsertion handle (Fig. 6), applying impactation until itis inserted to the required depth, at this point theinsertion handle is removed. With the final impactorthe definitive stem can be impacted in a complemen-tary way, if considered necessary (Fig. 7).
Finally we place the definitive head cosen on the stemand the articulation is reduced with the help of theimpactor-reducer (Fig. 8).
Nota: El mismo instrumental permite la colocacióntanto de un vástago KAREY HA (sin cemento) como elde un KAREY-C (con cemento) utilizando la mismatécnica descrita, teniendo en cuenta que para esteúltimo, se tendrán en consideración las pasosespecíficos de todos los vástagos cementados, como:limpieza de cavidades, preparación e inserción delcemento, obturadores dístales, etc.
Note: The same instrumentation is used to implantboth the KAREY HA (cementless) stem and the KAREY-C (cemented) stem using the same technic shown.However, take into consideration that for the implan-tation of the KAREY-C one must consider the stepsspecific to all cemented stems: cavity cleaning,preparation and insertion of the bone cement, distalobturators, etc.
