MANEJO INICIAL

1. Reducción de fracturas y luxaciones2. Posibilidad de reimplante de partes

amputadas3. Apósitos estériles y ferulaje temporal4. Iniciar antibióticos de amplio espectro:

1. Cefalosporina de 1º generación2. Aminoglucoóido

5. Profilaxis antitetánica

PLAN DE TRATAMIENTO

1. Clasificar lesiones y establecer pronóstico

2. Factores del paciente

3. Factores propios de la lesión

4. Factores que favorecen el reimplante vs la

amputación.

CONSIDERACIONES PREOPERATORIAS

• SISTEMA ESQUELETICO:– Fijación ósea: materiales

de osteosíntesis– Injertos óseos– Artroplastia y artrodesis

• VASCULAR:– Fasciotomias– Derivaciones vasculares

temporales– Reparación vascular directa– Injertos venosos

CONSIDERACIONES PREOPERATORIAS

• NERVIOSO– Injertos nerviosos

• TENDÓN– Injertos tendinosos

• PIEL:– Desbridamiento extenso– Injertos y colgajos– Sustitutos biológicos– Dispositivos de cierre al

vacio

MANEJO DEFINITIVO

• Recontrucción vs amputación.• Reconstrucción inmediata (colgajo libre de

urgencia) vs reconstrucción en etapas

• Rehabilitación

COMPLICACIONES

• Síndrome de compartimiento• Infección• Rigidez• No unión• Rabdomiolisis• Alteraciones psicológicas.
