  • • Antes de 5.0 Mónaco fue un sistema de planificación de tratamiento que usa el

    algoritmo de Montecarlo XVMC como motor de cálculo. Tiene la posibilidad de poder planificar IMRT/VMAT/DCAT.

  • Templates y Soluciones de Clase

    • Mónaco es un TPS que tiene de base los Templates – Templates = ahorran tiempo

    • Que significa esto? – Los templates almacenan toda la información relevante de

    un plan, que se puede aplicar para crear un plan nuevo en otro paciente.

    – Todos los datos desde geometría, parámetros de calculo y restricciones

  • • Las soluciones de clase son útiles por la siguiente razón: – Las geometrías de los pacientes en una población pueden variar, pero

    las relaciones entre los Target(s) y los OARs no varían tanto. – Las restricciones biológicas toman en cuenta todo el volumen del

    Target y los OARs por lo que las superposiciones de los volúmenes pueden variar, pero los restricciones no necesariamente.

    • Las soluciones de clase nos benefician por: – El uso de un template proporciona una forma mas estandarizada de

    planificar, donde el proceso es mas fácil de desarrollar y entender. – Una vez que este se ha usado en varios pacientes este puede ser una

    solución de clase para los casos, permitiendo tener una trazabilidad y consistencia en la creación del plan.

    Dependiendo la zona a tratar y los órganos de riesgo involucrados podemos tener: Estrategias de tipo:

    • Geométricas

    • Dosimétricas

  • ¿Arco completo o arcos parciales?

    En una primera aproximación hay que considerar las

    relación de los tamaños del PTV y OARs.

    •Los objetivos de los targets a veces no pueden ser reunidos.

    Etapa 1- Optimización de los constrains con la Fluencia de dosis Se optimiza la fluencia basado en la prescripción.

    Etapa 2- Optimización de las aperturas (segmentación ) Planes VMAT – Optimiza la forma de las aperturas, posiciones de Gantry y la ponderaciones de los segmentos para crear secuencias de arcos puedan ser entregados por el equipo.

    • EUD representa una dosis homogénea que cuando es aplicada al volumen blanco, tiene el mismo efecto clínico como si se diera una distribución de dosis heterogénea dentro del volumen blanco.

    • EUD permite puntos frios y calientes

    • Para OARs: EUD representa una dosis uniforme a la cual puede llevar a la misma probabilidad de daño como una distribución de dosis heterogénea en un OAR.

  • Función de Costo- Target EUD • EUD permite puntos fríos y calientes

    – Permite al optimizador mucha mas flexibilidad cuando se busca una buena optimización.

    **Función de Costo Biológica**

    – Puede ser controlado por la sensibilidad celular, aumentándola disminuye el volumen donde se permite estar por debajo de la prescripción, pero no se puede obtener dependiendo de otras restricciones de OARs.

    – Usando una restricción Max dose ayudará a una curva mas recta.
