Antecedentes
• Marzo 23, 2014: La OMS fue notificada de un brote de Enfermedad por virus del Ébola en Guinea.
• Agosto 8, 2014: La OMS declaró a la epidemia del Ébola ser “una emergencia de salud pública de preocupación internacional”
• Agosto 12, 2014: La OMS aprueba el uso de medicamentos experimentales para el tratamiento del Ébola

• Septiembre 14, 2014: se ha reportado un total de 4507 casos probables y confirmados, incluyendo 2296 muertes en 5 países (Guinea, Liberia, Nigeria, Senegal y Sierra Leona).

Virus del Ébola
• Los filovirus se encuentra entre los patógenos más virulentos causante de la enfermedad grave que semeja choque séptico fulminante.
• Tasa de letalidad aproximada del 70%.
• No hay terapia aprobada para el tratamiento.
• Actualmente en realización de nuevos enfoques experimentales para la profilaxis y tratamiento posterior a la exposición que son eficaces en primates de laboratorio.

Transmisión persona-persona
• Por contacto directo a través de piel lesionada y mucosa expuesta a fluidos corporales de personas quienes han desarrollado signos y síntomas de la enfermedad.
• Sangre, heces y vómito son los fluidos más contagiosos.
• Orina, semen, leche materna, saliva, lagrimas.
• Puede ser transmitido por superficies contaminadas y objetos.
• Raramente es transmitido por vía respiratoria.

• Estudio basado en la recolección de datos obtenidos en Guinea, Liberia, Nigeria y Sierra Leona.
• Se reportó un total de 4507 casos probables y confirmados de EVD en un periodo de 37 semanas.
a ) Caso sospechoso de EVD: Cualquier persona viva o muerta con fiebre alta repentina y antecedente de contacto con una persona con caso probable o confirmado de Ébola
ó
• Cualquier persona con aparición repentina de fiebre alta y al menos tres de los siguientes síntomas: cefalea, vómito, anorexia, diarrea, letargo, dolor abdominal, dolor muscular o articular, disfagia, disnea o hipo, o cualquier sangrado inexplicable.

• b ) Caso probable: Cualquier persona sospechosa de tener EVD que fue evaluado por un médico y que tenía una relación epidemiológica con una persona con un caso confirmado, pero que no tenía una prueba confirmatoria.
• c ) Caso confirmado: Cuando una muestra de la persona era positivo para virus del Ébola.

Diagnóstico
• Prueba de inmunoabsorción enzimática (ELISA)
• Pruebas de detección de antígenos
• Prueba de seroneutralización
• Reacción en cadena de la polimerasa con transcriptasa inversa (RT-PCR)
• Aislamiento del virus mediante cultivo celular.

Prevención y tratamiento
• No hay vacuna contra la EVD.
• Los casos graves requieren cuidados intensivos.
• Tampoco hay ningún tratamiento específico, aunque se están evaluando nuevos tratamientos farmacológicos.

Conclusiones
• La presente epidemia es excepcionalmente grande no por las características biológicas del virus sino por los atributos de la población afectada y por los esfuerzos insuficientes de control.
• Periodo de incubación: 11.4 días (Rango 2 a 21 d)
• El periodo de incubación implica el seguimiento de los casos contactos por 21 días.

Conclusiones
• Mortalidad: Guinea (70.7%), Liberia (72.3%), y Sierra Leona (69%)
• A menos que las medidas de control mejoren, se proyecta en estos tres países se llegara a informar miles de casos y muertes por semana.
• Se proyecta para Noviembre 2, 2014 el número de casos probables y confirmados:– Guinea 5740
– Liberia 9890
– Sierra Leona 5000
– Excediendo en total más de 20,000 casos
